According to the Pitt County Sheriff's Office, 54-year-old Sherman Chico Williams, of Raleigh, died in the accident Monday morning. It happened at the Weyerhaeuser mill at 371 E Hanrahan Rd., between Grifton and Ayden.
Investigators said a forklift fell on top of Williams, killing him.

Williams, who was employed as a machine operator, had worked at the Weyerhaeuser facility for the past seven-and-a-half years, according to Weyerhaeuser Public Affairs Manager Nancy Thompson.
"We have lost a valuable member of our Weyerhaeuser team and we extend our thoughts and prayers to his family," said Thompson in a statement Monday.
The mill, which employs 184 people, was closed following the accident.
The Occupational Safety & Health Administration (OSHA) is investigating.
An OSHA representative said the last time the plant was inspected was 2009. Based on information from the OSHA website, the facility had four violations that were listed as serious.
